STEPS FOR ISSUANCE OF STAFF IDENTIFICATION
NEW STAFF
Step 1
Individual Staff physically presents themselves at the ICT Media Office for photo shoot.
Step 2
Staff is issued with ID
REPLACEMENT OF LOST STAFF IDENTITY CARD
· Staff must inform the Human Resource Officer
· Affected staff MUST provide a police abstract to the PU Security office
· Security Office shall issue a Replacement of ID form;
Step 2 – Replacement of ID Form procedure
Staff shall fill and submit the duly filled formed to Human Resource office for verification and onward action to the ICT media Section for processing of Staff ID.
Step 3
Staff shall collect their new ID from the Media Service section within 24hrs
REPLACEMENT OF DAMAGED STAFF IDENTITY CARD
· Human resource office shall issue a Replacement of ID Form
CHANGE OF DESIGNATION
STEPS FOR ISSUANCE OF STUDENTS IDENTIFICATION
New Students
Individual Student shall physically present themselves at the ICT Media Office for photo shoot
Required before ID process done:
1. Fee Statement
Student is issued with ID
REPLACEMENT OF LOST/DAMAGED STAFF IDENTITY CARD
Lost Students ID
· Affected Student MUST provide a police abstract to the PU Security office
· Security Office shall issue a replacement of ID form;
Step 2 – Replacement Form procedure
Students shall
1. Fill form,
2. Pay Kshs. 500/- at the Finance Department and attach it to form,
3. Seek verification from the Admissions office,
Submit form with documentation as above to the ICT Media Services Section for action.
Once verified form is received, Student ID shall be processed, produced and issued to student.
DAMAGED STUDENTS ID
· Affected Student must alert the Security Office of a damaged Card
4. Fill form,
5. Pay Kshs. 500/- at the Finance Department and attach it to form,
6. Seek verification from the Admissions office,
7. Attach damaged ID card to form
In addition, submit to the ICT Media Services Section for action.
CHANGE OF COURSE
· Affected Student must alert the Security Office of change of course
1. fill form,
2. pay Kshs. 500/- at the Finance Department and attach payment receipt to form,
3. Seek verification from the Admissions office
4. Attach a copy of the change of course letter
5. Attach current ID card
Submit form with documents attached as above to the ICT Media Services Section for action.
